This is a unique Community Palliative Care Nurse role where you can make a real difference in the lives of our patients and their families across the West Auckland region.

If you thrive on complexity, diversity and love working with a dynamic team of multidisciplinary Health and Social care professionals, this may be the role for you. You can enjoy putting your ideas into action and can further explore ways to add improvement in nursing service delivery to the people we serve.

You will be a part of the wider team that thrives on delivering home consultations, patients' clinics, online telehealth and communicating and liaising with GP's, Residential Care and other community providers.

You will be helping us to extend our support to people and their whanau through the consistent delivery of comprehensive, compassionate, and responsive community specialist palliative care.

We have part time and full time roles available, with flexible working hours.

You will bring..., drive and passion to deliver Community Palliative Care differently into the West Auckland communities. You'll be keen to develop as a nursing professional in palliative care. You will have a minimum of 3 years of registered nursing experience in a hospice or in a palliative related field, e.g., gerontology, oncology, haematology, or district nursing.

You will hold a Current New Zealand Nursing Practicing Certificate, have high computer literacy and comfortable working online providing telehealth via different technology mediums.

As a curious, flexible individual, you will enjoy actively being involved to improve services and are open to exploring innovative ways that drive the best outcomes for our people and communities.
