Use your nursing expertise to influence better health outcomes – not just care for individuals, but shape the systems that support them. If you're a Registered Nurse ready to take the next step into a more strategic, community-focused, and values-driven role, this opportunity is for you.

About Us

Community Connections is a values-driven, community-based disability support provider operating across much of the North Island. Our vision is an inclusive Aotearoa where disabled people are fully participating citizens, barriers are removed, and everyone is respected for who they are.

We’re proud to be an Equal Employment Opportunities employer, committed to Te Tiriti o Waitangi, the UN Convention on the Rights of Persons with Disabilities, and the NZ Disability Strategy. We strongly encourage applications from people with lived experience of disability or mental health, Māori, Pasifika, LGBTQIA+ communities, and people from diverse backgrounds.

What the role offers

As Health Advisor, you’ll play a key role in improving health outcomes for the people we support. Reporting to the Quality and Practice Manager, you’ll guide and strengthen clinical practice across our organisation, helping ensure services are safe, inclusive, and person-centred.

You’ll lead health assessments, identify risks, and support frontline staff with practical clinical guidance. Your work will include planning, developing protocols, and collaborating on projects that meet compliance standards while driving long-term, positive change. This is a role where your expertise can truly shape lives, supporting individuals to achieve better health and greater choice in how they live.

About you

We’re looking for a Registered Nurse who’s ready to move beyond day-to-day clinical work into a role where you can shape practice, support teams, and drive better health outcomes for disabled people.

You’re the kind of person who gets stuck in—you research, problem-solve, collaborate, and bring people with you. You care deeply about equity and want to be part of a team that values diverse perspectives and lived experiences.

Essential requirements

  • NZ Registered Nurse with a current Annual Practising Certificate
  • Minimum of 2 years’ post-registration experience
  • Strong communication and relationship-building skills
  • High level of integrity, professionalism, and discretion
  • Full NZ driver’s licence and willingness to travel

Desirable

  • Background in the disability sector or community-based health services
  • Understanding of health and disability legislation and safeguarding practices
  • Experience in developing personalised support plans or contributing to policy and service improvement

Why join us

At Community Connections, you’ll join a dynamic, purpose-led team that values initiative, collaboration, and a good sense of humour. We’re all about supporting each other, celebrating wins, and making a real impact in the lives of disabled people.

If you’re ready to step into a more strategic role, love research and improvement, and enjoy working with people who genuinely care - we want to hear from you!!!!!

What we offer:

  • Competitive salary + company vehicle
  • Generous professional development fund
  • Flexible work arrangements in a fun, inclusive team

Please note: Applicants must have the legal right to work in NZ, hold a full driver’s licence, and be willing to complete Police and Oranga Tamariki checks.
